Program: Beyond the Hospital Walls: A Partnership to Improve Care for Persons Experiencing Homelessness
Speaker Bios:
Sarah A. Stella, MD is a hospitalist with 11 years of experience providing inpatient care at Denver Health. This inspired her to create a hospital-community partnership to improve hospital care and care transitions for persons experiencing homelessness. Dr. Stella is the Lead Academic Partner for this project and has led a number of initiatives at Denver Health focused on improving care for patients with complex medical and social needs, including homelessness. She is an Associate Professor of Medicine and Co-Director of the Health Equity Pathway at the University of Colorado.
Ellen E. Sarcone, MD is a hospitalist and Associate Clinical Professor of Medicine at the University of Colorado. She also directs the Health Equity Pathway in the Internal Medicine Residency Program, and is an academic partner and collaborator for this partnership.
Community Partners (Panelists):
Ms. Tammy Bellofatto is the Director of Vocational Services at the Colorado Coalition for the Homeless (CCH). She has a wealth of expertise with community advisory panels (CAPs), and is the community expert on the project CAP.
Ms. Christy Brooks is a Mom, Pastry Chef and member of the CAP who has taught us many important lessons about homelessness.
Gwynneth Johnson, RN is an RN Care Manager for Stout Street Health Center, focused on transitions of care. In this role she visits Stout Street patients while hospitalized to assess health goals, post discharge needs and to coordinate care with hospital staff to improve their transition back into the community.
Nenna Arnold, RN has been a nurse for 24 years and has worked all over the world, including the housing program at CCH for the last 3 years.
Jessica Holmes is a Respite Case Manager at CCH where she advocates for the needs of persons experiencing homelessness and complex medical needs.
Zeina Toure, LSW has been a social worker at Denver Health for 5 years. She has worked and/or volunteered at several organizations in Denver that serve the homeless population, including St. Francis Center, Denver Rescue Mission and Karis Community. Outside of work, she enjoys learning about her community, making music, and dancing.
Ms. Elisabeth Francis is Street Outreach Worker at St. Francis Center who provides care and compassion to those experiencing homelessness in Denver.
Mr. Randle Loeb is a member of our CAP who has taught us many valuable lessons about empathy, and who reminds us to “be still longing for home for the heart filled with compassion, forgiveness and simplicity”.
Tammy Clarke has served 7 years as the Wellness Director of St Francis Center to address the behavioral health needs of the day center’s homeless community.
